About ASQ-3 (Ages & Stages Questionnaires)
The Ages & Stages Questionnaires (ASQ-3) is a validated developmental screening tool designed to identify children who may benefit from further evaluation. It assesses development across five key domains from 2 months to 60 months of age.
Five Developmental Domains Assessed
Communication
Language development, receptive and expressive skills, ability to understand and produce sounds and words.
Gross Motor
Large muscle movements including sitting, standing, walking, running, jumping, and overall body coordination.
Fine Motor
Small muscle movements, hand-eye coordination, grasping, drawing, feeding, and manipulation of objects.
Problem-Solving
Cognitive skills, logical thinking, puzzle-solving, understanding cause-and-effect, and play skills.
Personal-Social
Social interaction, emotional development, self-care skills, and ability to follow rules and instructions.
How ASQ-3 Works
- Age-Specific: Different questionnaires for different age groups (4-60 months)
- Parent-Reported: Completed by parents or caregivers based on observation
- 30-Item Format: Typically 30 questions per age questionnaire
- Simple Scoring: Yes (10 points), Sometimes (5 points), Not Yet (0 points)
- Evidence-Based: High sensitivity and specificity for identifying developmental concerns
Understanding ASQ-3 Scores
Screening Categories
Each domain has specific cutoff scores that determine whether the child is:
- ✓ Above Cutoff (On-Track): The child is meeting developmental milestones in this domain
- ⚠ Close to Cutoff (Monitor): The child is approaching the cutoff and may need monitoring or intervention planning
- ✗ Below Cutoff (Concern): Further evaluation is recommended; consider referral for professional assessment
How Scores Are Calculated
- Yes = 10 points (child demonstrates the skill consistently)
- Sometimes = 5 points (child demonstrates the skill occasionally or inconsistently)
- Not Yet = 0 points (child has not yet demonstrated the skill)
Each domain score is the sum of responses to questions in that domain (typically 6 questions per domain).
Typical Domain Score Ranges (Example: 36-Month ASQ-3)
- Communication: 0-60 points | Cutoff: 34 points
- Gross Motor: 0-60 points | Cutoff: 34 points
- Fine Motor: 0-60 points | Cutoff: 30 points
- Problem-Solving: 0-60 points | Cutoff: 32 points
- Personal-Social: 0-60 points | Cutoff: 32 points

Frequently Asked Questions
What age range does ASQ-3 cover?
ASQ-3 is designed for children from 2 to 60 months (newborn to 5 years). Separate questionnaires are available for specific age ranges: 4, 8, 12, 16, 20, 24, 30, 36, 42, 48, 54, and 60 months.
Is this calculator a diagnostic tool?
No. ASQ-3 is a screening tool only. It helps identify children who may need further evaluation by professionals such as pediatricians, developmental psychologists, or early intervention specialists.
Can I use ASQ-3 if my child was born prematurely?
Yes! For children born prematurely, use "corrected age" (chronological age minus months premature) until age 3. This accounts for the developmental catch-up period.
What should I do if my child's score suggests concern?
Consult with your pediatrician or contact your local early intervention program. They can discuss next steps, which might include comprehensive developmental assessment or services.
How reliable and valid is ASQ-3?
ASQ-3 has strong psychometric properties with 91% sensitivity and 86% specificity. It's one of the most widely used and researched developmental screening tools internationally.
Can I use this tool for children in other countries?
ASQ-3 has been adapted and validated in 20+ languages and countries. However, cultural differences in developmental practices may affect interpretations—always follow local guidelines.
How often should ASQ-3 be administered?
It's recommended to use ASQ-3 at well-child visits (typically every 2-4 months) or as part of routine developmental surveillance. Annual screening is common in community and preschool settings.
What if results don't match my observations?
Parent-reported tools can vary based on reporting styles, context, and familiarity with developmental milestones. Share results with your pediatrician for professional interpretation and guidance.
